Incident reporting

To alert SAFEX of an incident when it occurs 

The Form below enables Members who have experienced an incident or near-event involving explosives to notify the Secretariat right away. The Form only requires the essential information necessary for SAFEX to issue an Incident Notice (IN) to Members. Once you have completed your investigation into the incident you may submit your Investigation Report (IR). 

1. Complete the form below. All fields marked with * have to be completed.
2. Indicate a valid e-mail address in the "Contact mail" field.
3. Click on the "Submit” button.
4. You will receive an acknowledgment by e-mail.
5. If the Secretariat requires any clarification, you will be contacted directly
6. The Secretariat will use this information to issue an Incident Notice to Members
7. Once the SAFEX Secretariat issues the Incident Notice, it will be included in the Database.
