To submit an Investigation Report
Obtain the Investigation Report Template by clicking on “Load the template” below.
Once the template has been downloaded, fill in the detail required for each of paragraphs 1 to 9 using the prompts provided. (The template is posted in Microsoft Word format to enable you to fill it in directly).
Kindly send the completed template as an e-mail attachment to the SAFEX Secretariat.
