Incident Notification

Members can now notify SAFEX of incidents online by completing the Notification of Incident form after accessing the restricted pages (Intranet) with their access codes.

SAFEX eLearning modules – ENAEX commits to contribute to the next module with a paper presented at the Salzburg Congress

At the recent SAFEX Congress in Salzburg, an update on the available eLearning modules, formats, translations, and its use had been given. Though starting early in the morning, we experienced high participation of attendees to listen in.

On the 2nd day of the congress, Maria Virginia Ramirez (‘Marivi’) gave a presentation with the title ‘Emanation of Nitrous Gases in [the] Nitration Process’ at ENAEX Rio Loa plant in Chile. The paper discussed the immediate and root causes and inconsistencies in the emergency procedures very openly. The slides showing the sequence of events and walking the audience through the analysis had a high grade of animation and graphic information in a very professional style.

Immediately after the presentation, the idea was born that this incident – which has multiple Management of Change (MOC) components in it – would be an ideal candidate for the next SAFEX eLearning module.

The author reached out to the presenter and Juan Andres Errazuriz – ENAEX’ CEO – to ask for authorization to use the paper and content for that purpose. Juan Andres’, who attended the Congress, response and approval were quick and very positive:

‘First of all, I want to congratulate you and all the people leading the different initiatives at Safex. You [all] have done a terrific job, that allows the industry to increase our safety standards and the sustainability of all the companies. The idea of the eLearning modules is outstanding, and I definitely support it.’

I on behalf of SAFEX give Juan Andres, Marivi and ENAEX full credit for that commitment.

At the end of the day, this it’s what the SAFEX idea is all about: Reporting and sharing of incidents and learnings to help prevent this happening to others or to repeat.

In this example, we can see the snowballing effect: A SAFEX member company is presenting an incident and its learnings to a broader audience at the Congress and – with the eLearning tool – we as SAFEX are making it available for all our members in a different format.
